About the position

Matrix Medical Network is seeking a Nurse Practitioner for PRN/Part Time Home Risk Assessments. In this role, you will conduct comprehensive health assessments for at-risk health plan members in their homes, skilled nursing facilities, and through telehealth visits. Each visit can last up to one hour, during which you will review and observe the member's current health status, medical history, medication adherence, social environment, and other potential risks. This thorough assessment provides invaluable insights into the member's overall health and well-being, which can often be overlooked during standard office visits. The goal of the Matrix Comprehensive Health Assessment is to enhance the quality of care and address multiple care gaps in a single visit. As a Nurse Practitioner, you will work collaboratively with a range of healthcare professionals, including physicians, case managers, social workers, and family members, to ensure comprehensive care for each patient. You will also be responsible for providing health education, making physician referrals, and documenting all findings and actions according to Matrix guidelines and protocols. The position offers flexible scheduling, allowing you to work during days, evenings, nights, and weekends, depending on your availability and the needs of the patients. Responsibilities

  • Conduct Adult/Geriatric Assessments, including medical history, diagnosis and treatment, health education, and physician referrals.
  • Perform case management referrals and follow-up, ensuring clear documentation according to Matrix guidelines and protocols.
  • Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and other appropriate medical personnel.
  • Collaborate with Primary Care Physicians (PCP) on patient education and provide follow-up care.
  • Provide services in various venues, including home visits, skilled facility visits, and telehealth visits.

Requirements

  • Master's Degree required or commensurate experience and satisfactory completion of NP licensure.
  • Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain it.
  • Board certified by the AANP, ANCC, or the AACCN in a Matrix approved specialty.
  • Current BLS, ACLS, or CPR certification.
  • Minimum of 1 year experience as a Nurse Practitioner.
  • Strong computer skills and familiarity with employee health/medical record software.
  • Excellent verbal and written communication skills with patients, clients, and colleagues.
  • Comfortable and flexible with frequent changes.
