About the position
The Nurse Practitioner (NP) position at Matrix Medical Network involves conducting comprehensive health assessments for at-risk individuals in their homes or other community settings. The role focuses on evaluating patients' health, medical history, and social environments to improve care quality and close care gaps. This part-time, PRN role offers flexible scheduling and the opportunity to make a significant impact on patients' health outcomes.
Responsibilities
- Conduct Adult/Geriatric Assessments, including medical history, diagnosis, treatment, health education, and referrals.
- Work collaboratively with physicians, case managers, social workers, family members, and other medical personnel.
- Provide follow-up care and clear documentation according to Matrix guidelines and protocols.
- Collaborate with Primary Care Physicians (PCP) on patient education and follow-up services.
- Deliver services in various settings, including home visits, skilled facility visits, and telehealth visits.
Requirements
- Master's Degree required or equivalent experience and completion of NP licensure.
- Current RN and NP licensure in the state of practice, including prescription authority or ability to obtain it.
- Board certified by AANP, ANCC, or AACCN in a Matrix-approved specialty.
- Current BLS, ACLS, or CPR certification.
- Minimum of 1 year experience as a Nurse Practitioner.
- Strong computer skills and familiarity with health/medical record software.
- Excellent verbal and written communication skills with patients, clients, and colleagues.
- Comfortable and flexible with frequent changes.
