About the position

The Nurse Practitioner position at Matrix Medical Network is a PRN/Part Time role focused on conducting home risk assessments for at-risk health plan members. The role involves visiting patients in their homes or other community settings to evaluate their health, medical history, and social environment, providing a comprehensive health assessment that aims to improve care quality and close care gaps. This position offers flexible scheduling and the opportunity to make a significant impact on patient health outcomes.

Responsibilities

• Conduct Adult/Geriatric Assessments, including medical history, diagnosis, treatment, health education, and documentation according to Matrix guidelines.
• Work collaboratively with physicians, case managers, social workers, family members, and other medical personnel.
• Provide follow-up care and education in collaboration with Primary Care Physicians (PCP).
• Deliver services in various settings, including home visits, skilled facility visits, and telehealth visits.

Requirements

• Master's Degree in Nursing or equivalent experience and completion of NP licensure.
• Current RN and NP licensure in the state of practice with prescription authority or ability to obtain it.
• Board certified by AANP, ANCC, or AACCN in a Matrix approved specialty.
• Current BLS, ACLS, or CPR certification.
• Minimum of 1 year experience as a Nurse Practitioner.
• Strong computer skills and familiarity with medical record software.
• Excellent verbal and written communication skills.
