About the position

Legacy Hospice is seeking a dedicated PRN Hospice Nurse Practitioner to provide exceptional patient care in the Winchester Area. This role involves evaluating health histories, diagnosing medical conditions, and developing personalized care plans while collaborating with patients, families, and healthcare practitioners to ensure a holistic approach to hospice care.

Responsibilities

• Provide comprehensive medical care, palliate, and manage terminal illnesses under the supervision of a collaborating physician.
• Consult with the patient's attending physician and hospice medical director to assess eligibility for hospice care and coordinate ongoing care needs.
• Review clinical information and oversee patient care in collaboration with the hospice interdisciplinary team.
• Engage in patient and family education and counseling, addressing medical, psychological, and spiritual concerns.
• Make recommendations regarding patient eligibility for initial certification and recertification for hospice care to the Hospice Medical Director.
• Maintain up-to-date knowledge of Medicare/Medicaid and state regulations for hospice services.

Requirements

• Master's degree with a minimum of one year hospice or palliative care experience as an RN or NP preferred.
• Current unencumbered license in the State of Practice as a Registered Nurse and Nurse Practitioner.
• Certification in a specialist area preferred (e.g., Hospice and Palliative Nursing (CHPN), Pediatrics, or Geriatrics).
• Excellent verbal and written communication skills.
• Computer proficiency in typing and various programs, including EMR.
• Possess and maintain CPR certification if required by state.
