About the position

The Nurse Practitioner at WelbeHealth plays a crucial role in providing comprehensive care to seniors through the PACE program. This position involves collaborating with physicians and an interdisciplinary team to assess, plan, and deliver primary care services, ensuring that participants receive holistic treatment tailored to their needs. The role emphasizes a commitment to treating the whole person and involves regular interaction with patients in various settings, including nursing homes and during hospital discharges.

Responsibilities

• Perform in-person comprehensive assessments including initial, semi-annual and unscheduled assessments and continuous medical coverage to participants
• Collaborate with physicians and IDT to develop care plans for each participant
• Collaborate with physicians and IDT in discharge planning from acute or long-term settings
• Help maintain care of participants in the nursing home, making regular visits as dictated by participant need
• Work collaboratively with WelbeHealth physicians, community-based PACE physicians, and IDT to oversee and ensure effective care coordination
• At the direction of the Medical Director, review outcomes and utilization data, including hospitalizations, and use of specialists to establish best practices and policy improvements
• Take part in rotating night and weekend call

Requirements

• Graduation from an Accredited School of Nursing
• Certificate as a Nurse Practitioner with Prescriptive Authority
• RN State License
• Active Drug Enforcement Administration (DEA) registration
• Must be currently enrolled or eligible for enrollment to provide services to Medicare and Medi-Cal participants
• Minimum of two years of experience working with the senior population
• Current knowledge of chronic care/geriatric issues and best practices
• Reliable means of transportation
