About the position

The Nurse Practitioner (NP) in Primary Care is a licensed healthcare provider responsible for delivering direct patient care, including assessment, diagnosis, and treatment of various health conditions. This role involves working collaboratively with physicians and other healthcare professionals to develop and implement comprehensive treatment plans, while also providing education and counseling to patients and their families. The NP operates within a hybrid model, balancing in-office and remote patient care, and is expected to manage a diverse patient panel effectively.

Responsibilities

  • Assess the physical and mental condition of patients through comprehensive examinations and medical histories.
  • Order, collect, interpret, and evaluate laboratory and diagnostic tests to assess patient health needs.
  • Prescribe and recommend medical therapies and perform procedures in accordance with approved protocols.
  • Formulate and implement comprehensive, outcomes-based patient treatment plans based on assessments and evidence-based medicine.
  • Evaluate patient responses to healthcare provided and the effectiveness of care.
  • Counsel patients and families regarding medical processes, health management, and nutrition.
  • Maintain detailed records of patient treatment plans and outcomes.
  • Initiate timely consultations and referrals when necessary.
  • Provide consultation services for conditions related to the provider's specialty.
  • Perform invasive procedures as delegated by the supervising physician.

Requirements

  • Current Texas RN licensure or RN licensure with compact privilege from a state in the Nurse Licensure Compact.
  • Current license to practice as a Nurse Practitioner in Texas.
  • Basic Life Support certification within 30 days of hire or transfer.
  • National Nurse Practitioner certification in area of specialty.
  • Less than 1 year of experience as a Nurse Practitioner.

Nice-to-haves

  • 5+ years of experience as an Advanced Practice Provider in family medicine.
  • Experience in managing virtual care and patient-centered approaches.
