About the position

The Wound Care Nurse Practitioner (NP) plays a crucial role in providing specialized wound care to patients in various healthcare settings, including long-term care, transitional care, assisted living, and memory care homes. This position involves collaborating with nursing teams to develop and maintain effective wound management plans, conducting on-site patient visits, and offering telehealth consultations. The NP is responsible for comprehensive patient assessments, wound debridement, and educating nursing staff and families on wound care practices to ensure optimal healing outcomes.

Responsibilities

• Provide wound management for patients in collaboration with facility nursing teams.
• Conduct on-site visits and telehealth consultations for wound care.
• Perform comprehensive patient assessments focusing on infection risk factors.
• Execute wound debridement and apply appropriate dressings.
• Educate nursing staff and maintain communication with primary care providers and families regarding wound care interventions.
• Document patient treatment plans within 24 hours of visits.
• Order and interpret diagnostic studies to modify treatment plans as necessary.
• Collaborate with multidisciplinary teams for bedside skin care procedures.
• Assist in identifying wound care quality measures and track documentation.

Requirements

• 2 to 3 years of wound care experience.
• Medicare Credentialed.
• Graduate of an accredited nursing program.
• Knowledge of wound care-related products and procedures.
• Strong organizational and analytical skills.
• Bilingual in English and Spanish.

Nice-to-haves

• Graduate of a Wound Ostomy Continence Nurse Practitioner Program.
• Certified Wound Care Nurse (CWCN) Certification, CWS or WCC.
• Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) preferred.
