Responsibilities
- Rounding and managing clinical care of the population at assigned SNF.
- Prescribing medications and treatments for residents at SNFs.
- Providing comprehensive primary care services to patients in SNFs.
- Timely documentation of all patient care encounters.
- Available for phone/secure messaging consultations with facility nursing staff.
- Completing documentation in Theoria's proprietary EHR system, ChartEasy™.
- Ensuring timely attendance for scheduled shifts.
- Maintaining reliable transportation.
Requirements
- Active Nurse Practitioner license in North Carolina.
- Experience in primary care or skilled nursing facilities preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Familiarity with electronic health record systems.

- Experience with PointClickCare, MatrixCare, or American HealthTech EHR systems.
