About the position

The Nurse Practitioner (NP) at WellBe plays a crucial role in delivering compassionate home-based medical care to elderly patients with chronic and acute illnesses. This position involves providing primary, urgent, and acute medical care, maintaining patient records, and collaborating with a multidisciplinary team to ensure comprehensive care. The NP is responsible for engaging patients in WellBe programs, performing screenings, managing medications, and delivering palliative care when necessary, all while adhering to company metrics and maintaining a positive work environment.

Responsibilities

  • Deliver primary, urgent, and acute medical care to patients in their homes.
  • Maintain accurate and current patient records.
  • Schedule initial, urgent, and follow-up appointments as required.
  • Engage attributed patients into WellBe programs.
  • Facilitate decision-making by providing educational resources to staff, patients, and families.
  • Perform initial patient screenings and physical examinations, creating appropriate assessment and care plans.
  • Address urgent care needs of patients as needed.
  • Collaborate with the community team to complete care assessments.
  • Order, interpret, and analyze diagnostic tests, determining the need for follow-up appointments and further treatment options.
  • Prescribe and manage medications and treatments as appropriate, including psychopharmacological agents.
  • Deliver quality care while maintaining documentation and coding metrics, HEDIS measures, and managing medical expenses.
  • Serve as a resource for patients, providing ongoing care information, counseling, and guidance for critical conditions.
  • Refer to internal/external resources as appropriate, including behavioral health protocols.
  • Collaborate with the interdisciplinary team and participate in regularly scheduled team meetings.
  • Maintain required licensure and certification.
  • Deliver palliative care and end-of-life care to patients and families.
  • Facilitate family meetings to address complicated dynamics and goals of care.

Requirements

  • Current Nurse Practitioner licensure and certification.
  • Experience in providing primary care, urgent care, and geriatric care.
  • Strong communication and interpersonal skills to work collaboratively with a multidisciplinary team.
  • Ability to perform physical examinations and screenings.
  • Knowledge of medication management and treatment protocols.
  • Familiarity with documentation and coding standards in healthcare.

Nice-to-haves

  • Experience in home-based care settings.
