About the position

The Nurse Practitioner position at Theoria Medical involves providing high-quality, patient-centered care primarily in skilled nursing facilities (SNFs). The role emphasizes daily rounding, medication management, and establishing meaningful connections with patients and staff. Responsibilities

• Manage clinical care of patients at assigned skilled nursing facilities (SNFs).
• Round daily and prescribe medications and treatments for residents.
• Provide comprehensive primary care services to patients in SNFs.
• Document all patient care encounters in the electronic health record (EHR) system.
• Be available for phone and secure messaging consultations with facility nursing staff.
• Ensure timely attendance for scheduled shifts.

Requirements

• Active Nurse Practitioner license in the state of practice.
• Experience in primary care or skilled nursing facilities preferred.
• Ability to use electronic health record systems, specifically ChartEasy™ and familiarity with PointClickCare, MatrixCare, or American HealthTech.

Nice-to-haves

• Experience with conversational and Dragon dictation for documentation.
• Strong communication skills for patient and staff interactions.
