About the position

Health Reach is seeking a dedicated Family Nurse Practitioner to provide comprehensive primary care services to individuals experiencing homelessness in Rochester, NY. This role involves delivering care in various settings, including clinics, shelters, and mobile units, while adhering to evidence-based practices and maintaining high standards of patient care.

Responsibilities

• Provide comprehensive primary care medical services for all ages in an ambulatory setting.
• Perform minor outpatient procedures in accordance with the scope of practice and privileging policy.
• Consult with clinic physicians, specialists, or other health care professionals as appropriate.
• Assess patient needs for specialty referrals, diagnostic and in-patient care and make appropriate referrals.
• Provide counseling and education regarding health maintenance, disease prevention, diagnosis, treatment, and follow-up during patient visits.
• Verify and document patient understanding of diagnosis, treatment, and follow-up recommendations.
• Dispense medications in accordance with applicable New York State policies.
• Participate in a shared call system with other providers.
• Assist with meeting clinic patient care team productivity and quality care standards.
• Participate in development and utilization of practice guidelines and procedures.
• Complete accurate and timely documentation in patient records and encounters within 24-48 hours after the patient visit.
• Maintain credentials essential for practice, including licensure, certification, and CME.
• Demonstrate respectful, professional behavior that supports a team-oriented work environment.
• Attend staff meetings and participate in agency committees and task force activities as required.

Requirements

• Graduated from an accredited medical program and experience in a family practice clinic setting.
• Minimum of two years' experience as a licensed Family Nurse Practitioner.
• Current licensure to practice as a Registered Professional Nurse & Nurse Practitioner in NYS.
• Ability to read, analyze, and interpret complex medical documents.
• Intermediate level ability with Electronic Medical Record (EHR) systems, Word, Outlook, and other required software programs.

Nice-to-haves

• Previous Nurse Practitioner experience or a combination of related experiences in progressively advancing nursing practice and leadership roles.
