About the position

The Nurse Practitioner at Buckeye Hospice and Palliative Care is responsible for providing comprehensive hospice services, ensuring that patients receive appropriate medical care and support during their end-of-life journey. This role involves collaboration with the interdisciplinary team to evaluate hospice eligibility, manage patient care, and address symptom control, while also participating in clinical and administrative functions to enhance the quality of care provided.

Responsibilities

  • Complement attending physician care and act as a medical resource person to the interdisciplinary group (IDG).
  • Perform face-to-face visits as needed and evaluate hospice eligibility in consultation with the IDG.
  • Ensure patients receive appropriate measures to control uncomfortable symptoms.
  • Collaborate with the IDG to meet patient needs and support clinical and administrative staff in providing compassionate care.
  • Review clinical information and coordinate patient care with the hospice interdisciplinary team and other healthcare practitioners.
  • Provide direct medical services to patients in the absence of the attending physician.
  • Attend IDG conferences and participate in plan of care development.
  • Manage oversight of medications and treatment, documenting care provided in the patient's clinical record.
  • Maintain current knowledge of research and trends in hospice care and pain/symptom management.
  • Participate in performance improvement programs and develop treatment protocols.

Requirements

  • Bachelor's degree in nursing or a related field.
  • Current Nurse Practitioner license in the state of Ohio.
  • Experience in hospice or palliative care settings is preferred.
  • Strong communication and counseling skills for dealing with end-of-life issues.
  • Ability to perform physical assessments and demonstrate patient care.
