About the position

The Nurse Practitioner at WellBe Senior Medical plays a crucial role in delivering home-based medical care to elderly patients with chronic and acute conditions. This position focuses on providing compassionate, concierge-level care in the patients' homes, ensuring they receive the necessary medical attention and support. The Nurse Practitioner collaborates with a multidisciplinary team to manage patient health, maintain accurate records, and facilitate care across various settings, including hospitals and skilled nursing facilities.

Responsibilities

  • Provide primary, urgent, and acute medical care to patients in their homes.
  • Engage patients in the WellBe program and manage their health and wellbeing.
  • Perform initial patient screenings and physical examinations, creating appropriate assessment and care plans.
  • Address urgent care needs of patients as required.
  • Collaborate with the interdisciplinary team to complete care assessments and participate in team meetings.
  • Order, interpret, and analyze diagnostic tests, determining the need for follow-up appointments and further treatment options.
  • Prescribe and manage medications and treatments, including psychopharmacological agents.
  • Update patient records and ensure accuracy at each appointment, maintaining compliance with company metrics.
  • Deliver palliative care and end-of-life care when needed, facilitating family meetings to discuss goals of care.
  • Serve as a resource for patients, providing ongoing care information, counseling, and guidance.

Requirements

  • Minimum Master's Degree in Nursing.
  • Valid nurse practitioner license in the state of operations and current board certification in the field.
  • 5+ years of progressive care experience required.
  • BLS certified and possess an NPI# as a Nurse Practitioner.
  • Valid driver's license, car insurance, and access to a car for patient visits.

Nice-to-haves

  • Home care experience preferred.
  • Strong computer skills, including proficiency in Word, Excel, and PowerPoint.
