About the position

The Nurse Practitioner at VIA Health Partners plays a crucial role in providing high-quality end-of-life care to patients with advanced life-threatening or life-limiting illnesses. This position emphasizes a people-first approach, ensuring that care is accessible to all patients and families, regardless of their financial situation. The Nurse Practitioner will work closely with patients and their families to manage symptoms, provide education, and support during the end-of-life process, while also contributing to the organization's mission of compassionate care.

Responsibilities

  • Promotes the highest quality of end-of-life care including hospice care and advanced directives.
  • Prevents suffering and alleviates physical, spiritual, and psychological pain.
  • Diagnoses, treats, and manages pain and other symptoms experienced by patients at the end of life.
  • Guides and counsels patients and families regarding disease progression and therapeutic measures.
  • Prescribes therapeutic measures, tests, procedures, and drugs in accordance with ethical principles.
  • Plans for situations beyond expertise and confers with the Medical Director or Medical Services Physician as appropriate.
  • Serves as an end-of-life care education resource within the organization and community.
  • Evaluates patients by performing history and physical examinations.
  • Conducts patient/family meetings to review medical issues and establish goals of care.
  • Provides general psychosocial support and refers for diet/exercise therapies.
  • Recommends non-pharmacological therapies and additional psychosocial counseling.
  • Prescribes medical supplies and equipment and recommends appropriate diagnostic tests.
  • Reviews and discusses advance care directives and recommends referral to hospice services.
  • Participates in team discussions regarding patient care and self-directs learning to stay updated on trends in hospice and palliative medicine.

Requirements

  • Master's Degree in nursing.
  • Current, active, and unrestricted NC license as Registered Nurse (RN).
  • Official recognition as Nurse Practitioner from NC Board of Nursing.
  • Successful board certification in specialty within 2 years of hire.
  • Current and unrestricted DEA and Basic Life Support certification required.
  • Computer proficiency is required.
  • Minimum 1 year related experience in direct care of patients with life-limiting illnesses.

Nice-to-haves

  • Experience in hospice and palliative care, family medicine, internal medicine, geriatrics, oncology, or home health care.
