About the position

HealthDrive is seeking a Nurse Practitioner to manage the medical needs of residents in skilled nursing facilities, focusing on both sub-acute rehabilitation and long-term care. The role involves diagnosing and treating health conditions, promoting health practices, and collaborating with medical staff. The Nurse Practitioner will serve as the primary contact for emergencies and routine medical visits, conducting physical examinations, interpreting lab tests, and overseeing patient health. HealthDrive offers a comprehensive training program to prepare Nurse Practitioners for success in their roles.

Responsibilities

  • Provide primary, post-acute, and preventative care for residents in both the sub-acute rehab and long-term care units.
  • Order and study diagnostic tests (blood tests, X-rays, etc.).
  • Diagnose health conditions and illnesses.
  • Propose treatments for chronic and infectious diseases.
  • Prescribe medications.
  • Maintain accurate records and schedules.
  • Inform patients about their health conditions and prescribed medications.
  • Oversee patients' overall health.
  • Be on-call only 2 times per month from 6pm-8am.

Requirements

  • Master's Degree from an accredited college/university.
  • State professional Nurse Practitioner license.
  • Board Certification Required.
  • DEA/Controlled Substance License.
  • Valid driver's license.
  • Ability to work both independently and collaboratively with on-site staff.
  • Excellent written, verbal, interpersonal and organizational skills.
  • Ability to use email, the internet, and to learn other healthcare related software.

Nice-to-haves

  • Experience in one of these Nurse Practitioner specialties: gerontology, acute care, cardiology, internal medicine, or endocrinology.
  • Nursing experience in ICU/ER/SNF.
