Responsibilities

• Manage clinical care of the population at assigned SNFs.
• Round daily and prescribe medications and treatments for residents.
• Provide comprehensive primary care services to patients in SNFs.
• Timely documentation of all patient care encounters.
• Available for phone/secure messaging consultations with facility nursing staff.
• Complete documentation in Theoria's proprietary EHR system, ChartEasy™.
• Be on-site and punctual for scheduled shifts.

Requirements

• Current Nurse Practitioner licensure in the state of Vermont.
• Experience in primary care or geriatric care preferred.
• Ability to manage patient care in a skilled nursing facility setting.
• Strong communication skills for patient and staff interactions.
• Reliable transportation to travel to various facilities.
