About the position

The Nurse Practitioner at Hospice of North Alabama plays a crucial role in providing hospice care to patients. This position involves evaluating, assessing, diagnosing medical conditions, developing care plans, and prescribing necessary treatments. The Nurse Practitioner is integral to ensuring patients receive compassionate and effective care, making a positive impact from the very first day.

Responsibilities

  • Providing medical care, palliation, and management of terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician.
  • Completing history and physical examinations by evaluating the patient's medical condition and health history.
  • Completing the hospice face-to-face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility.
  • Making recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care.
  • Completing and maintaining accurate clinical notes.
  • Communicating with the physician and participating in interdisciplinary group meetings to meet the medical and emotional needs of the patient and family/caregivers.
  • Ensuring compliance with all state and federal legal and regulatory requirements.
  • Additional responsibilities as assigned.

Requirements

  • Graduate from an accredited School of Nursing and accredited Nurse Practitioner program.
  • Current DEA license, if applicable to state regulations.
  • Current nursing license in state of practice.
  • Current CPR certification.
  • Master's degree with a minimum of one year NP experience.
  • One year hospice or palliative care experience preferred.
  • Accreditation from an approved certifying body for advanced practice nursing as required by state of practice.
  • Certification in a specialist area preferred (e.g. Hospice and Palliative Nursing (CHPN) or Geriatrics).
  • Commitment to clinical and documentation excellence.

Nice-to-haves

  • One year hospice or palliative care experience preferred.
  • Certification in a specialist area preferred (e.g. Hospice and Palliative Nursing (CHPN) or Geriatrics).
