About the position

Theoria Medical is seeking dedicated physician assistants (PAs) and nurse practitioners (NPs) to provide high-quality care at skilled nursing facilities (SNFs). This part-time, in-person role involves managing clinical care, prescribing medications, and ensuring timely documentation while working in a flexible and positive environment. The position requires in-person rounding primarily on weekends and holidays, with the opportunity to complete charting off-site.

Responsibilities

• Rounding and managing the clinical care of the population at the assigned SNF.
• Prescribing medications and treatments for residents at SNFs.
• Providing high-quality, patient-centered, comprehensive primary care services to patients residing in SNFs.
• Timely documentation of all patient care encounters.
• Available for phone/secure messaging consultations with facility nursing staff.

Requirements

• Valid PA/NP license in the state of practice.
• Valid NP/PA degree from an accredited institution.
• Active DEA license (assistance provided for new graduates).
• General post-acute/SNF knowledge or experience preferred.
• Familiarity with EMR systems and charting practices.

Nice-to-haves

• Experience in post-acute care settings.
• Familiarity with Theoria's proprietary electronic health record (EHR) system, ChartEasy™.
