About the position

The Nurse Practitioner (NP) at ChenMed/Jencare plays a crucial role in providing direct patient care across various settings, including medical centers, nursing homes, skilled nursing facilities (SNF), and home environments. This position is integral to the clinical operations team, focusing on geriatric assessments, medical histories, physical examinations, diagnoses, and treatment plans. The NP is responsible for developing comprehensive nursing care plans, providing health education, and making necessary referrals for case management. The role requires adherence to ChenMed's standards for quality, service, productivity, and teamwork, ensuring that all documentation is thorough and compliant with regulatory guidelines. In addition to direct patient care, the Nurse Practitioner participates in clinical rounds and conferences, contributing to the overall quality of care provided to patients. The NP must demonstrate the ability to work both independently and collaboratively with other healthcare professionals, consulting with managers, physicians, and medical directors to ensure compliance with established guidelines. This position may also involve participation in risk and quality management programs, clinical meetings, and other relevant activities. The NP is expected to assess acute and non-acute clinical problems independently, document physical assessments and patient histories, and analyze trends in patient conditions. They will develop and implement patient management plans based on the data collected, while also providing counseling and education to patients and their families. The role includes writing admission, transfer, and discharge orders, ordering and interpreting laboratory and diagnostic studies, and prescribing medications as per state regulations. The Nurse Practitioner serves as a patient advocate, ensuring that care is tailored to the individual needs of each patient, considering their cultural backgrounds and support systems. This position is vital in enhancing the quality of healthcare for seniors, aligning with ChenMed's mission to transform healthcare for the better.

Responsibilities

  • Provide direct patient care in various settings including medical centers, nursing homes, and home environments.
  • Conduct geriatric assessments, medical histories, and physical examinations.
  • Develop and implement nursing care plans based on patient assessments.
  • Provide health education and make referrals for case management as needed.
  • Document patient care and progress notes according to ChenMed standards.
  • Participate in clinical rounds and conferences to discuss patient management strategies.
  • Collaborate with multidisciplinary teams to ensure effective patient care.
  • Recognize and respond to situations requiring immediate physician attention.
  • Facilitate patient/family conferences to review treatment goals and optimize resource utilization.
  • Prescribe medications based on state regulations and patient needs.

Requirements

  • ARNP or similar advanced degree in Nursing required.
  • Current Nurse Practitioner Certification in the State of practice required.
  • Board certification by AANP or ANCC preferred but may be required for certain states.
  • Current DEA number from the DEA for schedule II-V controlled substances may be required based on state of practice.
  • A minimum of 1 year of clinical experience preferred, but strong new graduates will be considered.
  • Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required within the first 90 days of employment.

Nice-to-haves
