Responsibilities

- Providing comprehensive primary care services to patients in SNFs.
- Timely documentation of all patient care encounters in the electronic health record system.
- Available for phone/secure messaging consultations with facility nursing staff.
- Ensuring on-time attendance for scheduled shifts.
Requirements
- Active Nurse Practitioner licensure in the state of practice.
- Experience in providing primary care services, preferably in a post-acute setting.
- Ability to document patient care efficiently using electronic health record systems.
