About the position

The Judge Group is seeking two Nurse Practitioners to conduct in-home health risk assessments in Winchester and Frederick, Virginia. This role is with one of the largest healthcare providers in the country, offering a unique opportunity to engage with patients in their own environments. The Nurse Practitioners will be responsible for performing comprehensive health assessments, which include taking medical histories and conducting physical examinations either in the patient's home or occasionally in nursing homes or other healthcare facilities. Telehealth assessments may also be part of the role, providing flexibility in how care is delivered. The position is designed to be flexible, allowing Nurse Practitioners to work part-time, approximately 2-3 days a week, based on their availability and the needs of the patients. Pay rates are highly competitive and are structured on a per-assessment basis, making this an attractive opportunity for those looking to balance work with other commitments. Weekend work is permitted, further enhancing the flexibility of this role. In addition to conducting assessments, the Nurse Practitioners will be responsible for clinical documentation of both acute and chronic healthcare issues using an electronic medical record (EMR) system. They will ensure that all necessary consent forms are signed and dated, and will provide education to patients and their families regarding chronic medical conditions, preventative care, and medication adherence. Referrals to case management, nursing, pharmacy, and other services will be made as needed, and the Nurse Practitioners may also need to obtain samples for diagnostic tests, such as finger sticks for HgbA1C and LDL levels, as well as urine analyses based on client needs. The role also involves reviewing patient records for past and present health issues, conducting activities of daily living (ADL) and fall risk evaluations, depression and nutritional health screenings, BMI evaluations, and medication reconciliations. This comprehensive approach ensures that patients receive holistic care tailored to their individual health needs.

Responsibilities

  • Perform in-home health risk assessments including history and physicals.
  • Conduct assessments in nursing homes or other health facilities, and via telehealth as needed.
  • Document clinical findings in the EMR system for acute, chronic, and long-term healthcare issues.
  • Ensure all appropriate consent forms are signed and dated.
  • Educate patients and their families on chronic medical conditions, preventative care, and medication adherence.
  • Complete referrals to case management, nursing, pharmacy, etc. as needed.
  • Obtain samples for diagnostic screening tests such as finger stick for HgbA1C, LDL, and lead levels; urine analysis may also be required.
  • Review patient records for past and present health issues.
  • Conduct ADL and fall risk evaluations, depression and nutritional health screenings, BMI evaluations, and medication reconciliations.

Requirements

  • Family Nurse Practitioner (NP) required; Adult NP may be considered if no pediatric patients will be seen.
  • Must be board certified by the ANCC or AANP.
  • Must have an active/unrestricted NP license.
  • New graduates will be considered.
  • Must be able to be credentialed by Medicare, Medicaid, and other private insurance companies.
  • Must have use of own car with adequate automobile insurance.
