The position involves daily rounding, medication management, and maintaining timely documentation using proprietary electronic health record systems.

Responsibilities

• Rounding and managing clinical care of the population at assigned SNFs.
• Prescribing medications and treatments for residents in SNFs.
• Providing comprehensive primary care services to patients in SNFs.
• Timely documentation of all patient care encounters.
• Available for phone/secure messaging consultations with facility nursing staff.
• Completing documentation in Theoria's proprietary EHR system, ChartEasy™.

Requirements

• Active Nurse Practitioner licensure in the state of practice.
• Experience with PointClickCare or similar EHR systems.
• Ability to provide high-quality, patient-centered care.
• Reliable transportation to and from work sites.

Nice-to-haves

• Experience in skilled nursing facilities (SNFs).
• Familiarity with MatrixCare and American HealthTech EHR systems.
