About the position

Optum is seeking a Nurse Practitioner to join the Home-based Medical Care team in Benton County, WA. This part-time role focuses on providing home-based medical care to complex, chronic patients, particularly older adults, helping them maintain their health and well-being in their own homes. The position involves establishing relationships with patients and caregivers, conducting comprehensive assessments, developing care plans, and ensuring continuity of care through follow-up visits and collaboration with other healthcare providers.

Responsibilities

  • Establish and maintain effective relationships with patients, caregivers, and legal representatives.
  • Perform initial comprehensive assessments on newly enrolled patients and provide ongoing care.
  • Develop and update patient management care plans as needed.
  • Consult with Physician Leaders and coordinate with PCPs in care decisions.
  • Perform basic procedures of episodic nursing care, including IV placement, blood draws, and wound care.
  • Conduct post-discharge visits to ensure patient stability.
  • Participate in HEDIS measure campaigns and quality initiatives.
  • Participate in clinical after-hours on-call program on a rotational basis.

Requirements

  • Certification through the American Academy of Nurse Practitioners (AANP), ANCC, or NCCPA.
  • Current, unrestricted NP or PA licensure in Washington state.
  • Active, unrestricted RN licensure in Washington state.
  • Access to reliable transportation.

Nice-to-haves

  • 2+ years clinical experience in primary care, home-based medical care, geriatrics, or similar settings.
  • Knowledge of long-term care and community resources.
  • Familiarity with CPT, ICD-10, and HCPC codes.
  • Understanding of clinical standards of care.
  • Awareness of NCQA requirements and CMS guidelines.
