The Full-Time Nurse Practitioner position involves direct patient physical assessment, education, and health planning. The role requires maintaining continuity of care through regular patient follow-up and assisting in the coordination of services with other disciplines and the patient's insurance plans. The Nurse Practitioner will advocate for patients in arranging medically necessary services and communicate with community resources to ensure adequate assistance for patients in need. Referrals to specialty services will be made when necessary, and the Nurse Practitioner will consult with collaborating physicians and other members of the healthcare team to establish a comprehensive plan of care. Participation in the quality assurance process is essential to assure optimal care delivery, and the Nurse Practitioner will provide comprehensive case management services with an emphasis on health promotion and disease prevention. Monitoring patient outcomes in accordance with practice protocols and collaborating with staff nurses and nursing administrators in patient care planning are key responsibilities. The Nurse Practitioner will also act as a resource for nursing staff, conduct education in service on practice issues, and perform other duties as assigned.
