About the position

The Nurse Practitioner position at Theoria Medical is designed for healthcare professionals who are passionate about providing high-quality, patient-centered care in skilled nursing facilities (SNFs). This role involves rounding and managing the clinical care of residents, ensuring that they receive comprehensive primary care services. The Nurse Practitioner will be responsible for daily rounds, prescribing medications, and documenting all patient encounters in a timely manner using Theoria's proprietary electronic health record (EHR) system, ChartEasy™. Familiarity with other EHR systems such as PointClickCare, MatrixCare, and American HealthTech is beneficial but not mandatory. The position offers a flexible schedule with 6-hour shifts from 7 AM to 1 PM, Monday through Friday, allowing for a balanced work-life dynamic. There are no on-call duties or overnight requirements, providing a supportive work environment that values the well-being of its staff. Responsibilities

• Rounding and managing the clinical care of the population at the assigned SNF.
• Prescribing medications and treatments for residents residing at SNFs.
• Providing high quality, patient-centered, comprehensive primary care services to patients residing in SNFs.
• Timely documentation of all patient care/encounters.
• Available for phone/secure messaging consultations with facility nursing staff.
• Completing provider documentation in Theoria's proprietary electronic health record (EHR) system, ChartEasy™.
• Being on-site and on time for scheduled shifts.

Requirements

• Active Nurse Practitioner license in the state of Georgia.
• Experience in primary care or skilled nursing facilities preferred.
• Strong communication skills for patient and staff interactions.
• Ability to document patient care efficiently using EHR systems.

• Previous experience in a similar healthcare setting.
