About the position

The Nurse Practitioner (NP) at Eau Claire Cooperative Health Center plays a vital role in delivering primary, preventive, and episodic care as part of a multi-disciplinary health care team. This position focuses on assessing patients' unique health statuses and promoting overall health and wellness. The NP will engage in quality improvement activities and collaborate with other healthcare professionals to enhance community health outcomes.

Responsibilities

  • Obtain and document thorough medical histories and conduct physical exams as appropriate.
  • Complete assessments and care plans considering psychosocial and social determinants of health.
  • Accurately complete superbills for billing purposes.
  • Review, complete, and sign off on all required documentation within expected timeframes.
  • Stay updated on advancements in the specialty by attending seminars and conferences.
  • Complete medical records in a timely manner as per policy.
  • Respond to messages from support staff and other team members.
  • Attend Cooperative meetings as scheduled, aiming for 75% attendance.
  • Coordinate plans to improve population health and achieve quality measure targets.
  • Make timely medical diagnoses based on history and physical exams.
  • Order appropriate tests to evaluate medical situations.
  • Prescribe medications with detailed administration instructions.
  • Provide necessary encounter information for billing, including CPT and ICD-10 codes.
  • Interpret clinical data and monitor the effectiveness of care plans.
  • Direct coordination of patient care with patients and clinical team members.
  • Engage, educate, and empower patients regarding their health.
  • Provide medical supervision to support staff in implementing care plans.
  • Serve as a preceptor for professional students during clinical rotations.
  • Establish effective working relationships with public and private clinicians and referral sources.
  • Work in the Saturday Walk-In office as arranged by the Division Director or CMO.
  • Perform other duties as assigned by the CMO and CEO.

Requirements

  • Must have successfully completed a master's or doctorate-level nursing program.
  • Must be certified by a nationally accredited credentialing agency.
  • Current licensure as RN in the state of South Carolina.
  • Certified by an appropriate credentialing organization recognized by South Carolina as a Nurse Practitioner.
  • Maintains currently signed protocols reviewed and revised annually and as needed.
  • 2 years of experience working in a clinical setting, preferred.

Nice-to-haves

  • Experience in Microsoft PowerPoint is a plus.
