About the position

The Nurse Practitioner position involves providing comprehensive care to patients, particularly in a nursing home setting. The role includes assessing patient health, diagnosing conditions, and administering treatments while collaborating with other medical professionals as needed. This full-time position offers a structured day shift schedule from Monday to Friday.

Responsibilities

• Document items such as past medical, family, and social history, review of systems, examinations, medications, allergies, assessment and plan.
• Order or execute various tests, analyses, and diagnostic images to provide information on the patient's condition.
• Analyze reports and findings of tests and examinations, and diagnose conditions.
• Administer or prescribe treatments as necessary.
• Order lab and imaging tests to determine and manage an immediate treatment plan.
• Vaccinate patients to immunize them from communicable diseases.
• Advise patients concerning diet, hygiene, and methods for prevention of disease.
• Consult with appropriate medical specialists when necessary and refer patients to subspecialists within the network.
• Provide care to patients in the nursing home.

Requirements

• Master's Degree in a Nurse Practitioner Program: Family or Adult Geriatrics.
• Current New York State License as a Nurse Practitioner.
• National Board Certification required from AANP or ANCC.
• Bilingual (Spanish Speaking) is a plus.
• EMR/EHR experience is a plus.
