The position involves overseeing the development of the hospice program in collaboration with senior leadership. Additionally, the role includes determining patient eligibility for General Inpatient care, managing the admission process, and developing patient care plans that involve both patients and their families. The position requires managing patient symptoms, administering medications, and participating in interdisciplinary team meetings and quality improvement programs. The candidate must consistently provide service excellence to all stakeholders and perform other duties as assigned.

Responsibilities

  • Oversee hospice program development including staff hiring and education.
  • Determine staffing needs based on census requirements.
  • Ensure compliance with personnel policies.
  • Review performance and conduct evaluations of direct reports.
  • Monitor compliance with regulatory and reimbursement requirements.
  • Maintain compliance with Joint Commission, state, and federal regulations.
  • Assist with staff orientation and ongoing education.
  • Collaborate with departments to develop and monitor hospice policies.
  • Determine patient eligibility for General Inpatient level of care.
  • Perform the admission process and establish patient plans of care.
  • Initiate and regularly re-evaluate the General Inpatient Plan of Care.
  • Manage patient symptom management and administer prescribed medications.
  • Attend Interdisciplinary Group Team meetings regularly.
  • Participate in in-service training and mandatory trainings.
  • Ensure availability of necessary equipment and services for patients.
  • Provide clinical expertise and serve as a resource to staff and the community.
  • Participate in Agency Quality Improvement program.
  • Provide service excellence to patients, families, and co-workers.

Requirements

  • Registered Nurse currently licensed in the Commonwealth of Massachusetts.
  • BSN, Master's degree in Nursing preferred.
  • Minimum of two to four years of supervisory experience in a home health agency, hospice or related health program.
  • Ability in program administration.
  • Excellent oral and written communication skills.
  • Ability to utilize available hardware and software.
