About the position

The Nurse Support Program provides essential nurse home visitation services to families referred by Adams County Human Services. This program is designed to support families and help them stay together by providing comprehensive nursing assessments, education, support, and resources. The nurse utilizes family-centered goals to promote self-sufficiency, enhance healthy caregiver-child interactions, and reduce further county involvement. The nurse is responsible for conducting nursing assessments and interventions while meeting program goals. The Professional Public Health Nurse for the Nurse Support Program (NSP) is a unique, relationship-based public health nursing role that requires the nurse to establish and maintain a trusting therapeutic relationship with each client throughout all phases of the program. The nurse assesses the physical, emotional, social, and environmental needs of the family and provides health education, referrals, and case management. This position fosters a relationship between the nurse and client as a basis for proactive change, working with families referred to the Program by Departments of Human Services. The NSP Nurse Home Visitor PHN I is an entry-level position requiring a Bachelor of Nursing degree and up to two years of nursing practice, while the NSP Nurse Home Visitor PHN II is a mid-level position requiring more than two years of nursing experience and advanced nursing process skills.

Responsibilities

  • Provide nursing assessments and interventions to families referred by Adams County Human Services.
  • Establish and maintain trusting therapeutic relationships with clients throughout all phases of the program.
  • Assess the physical, emotional, social, and environmental needs of families and provide health education, referrals, and case management.
  • Promote health maintenance through teaching, conducting needs assessments, and resource coordination.
  • Prioritize multiple work assignments, including home visits and timely completion of required documentation.
  • Develop and maintain therapeutic relationships with a culturally diverse client population.
  • Maintain an active case load and monitor client engagement in the program.
  • Collaborate with community agencies and service providers for client referrals and case management.
  • Represent Adams County as a health resource in the community and promote public health programs.
  • Complete clinical documentation and assessments accurately and in a timely manner.

Requirements

  • Bachelor's degree in nursing from an accredited college or university.
  • For PHN I: Minimum of one year nursing experience, preferably in Maternal Child Health and community-based services.
  • For PHN II: Minimum of three years nursing experience, preferably in Maternal Child Health and community-based services.
  • Active Colorado Nursing license and current CPR certification.
  • Up-to-date immunization status, including COVID.
  • Valid driver's license and access to dependable transportation for business purposes.
  • Proficiency in online meeting platforms and Microsoft Office tools.
