About the position

The Nursing Assistant-Cert-Hospice position at Southcoast Health involves providing compassionate care to patients in various settings, including their homes, skilled nursing facilities, and hospitals. The role focuses on assisting with daily living activities, ensuring patient comfort, and supporting families during challenging times. This position is integral to the hospice program, emphasizing a community-focused approach to healthcare.

Responsibilities

• Perform patient care activities (ADLs and IADLs) in patients' residences according to the Plan of Care.
• Assist with light housekeeping and meal preparation.
• Collect weight and vital signs, and assist in feeding patients.
• Provide respite and support for patients participating in activities programs.
• Offer personal care for actively dying patients and assist in post-mortem care.
• Communicate pertinent patient information and unusual findings to clinical personnel.
• Complete all required documentation accurately and in a timely manner.
• Deliver exceptional and caring customer service to patients and their families.

Requirements

• Completion of four years of high school plus specialized courses and training.
• CNA Certification required.
• CPR certification required.
• Three months of related work experience is required.
• Demonstrated effective interpersonal, customer service, and communication skills.
• Completion of initial 8-hour training for caring for dementia patients and 4 hours annually required.
• Differential pay applied if currently certified as or upon completion - CHPNA (certified hospice and palliative nursing assistant).
• Must be fully vaccinated against seasonal Influenza and COVID-19 or provide an exemption.
