About the position

The RN Clinical Supervisor at LifeSpring In-Home Care is responsible for overseeing a team of healthcare professionals to ensure quality care is provided to patients in their homes. This role requires clinical expertise, effective communication with healthcare providers, and strong time management skills to monitor care plans and support patients and their families.

Responsibilities

• Monitor and evaluate the effectiveness of care plans
• Communicate with collaborating physicians or specialists regarding patient care and changes in condition
• Educate patients about health maintenance and disease prevention
• Facilitate referrals to other healthcare professionals and medical facilities
• Ensure the accuracy of patient medical records
• Provide advice and emotional support to patients and their family members
• Other duties as assigned

Requirements

• Active, unencumbered RN license
• Mid-level supervising experience
• Experience in patient care and home health
• Ability to manage care plans
• Strong time management skills
