About the position

The Outpatient Nutritionist at Rejuven Health is responsible for assessing patients' dietary needs, creating individualized nutrition plans, and providing ongoing support to help clients improve their health. This role requires a compassionate and experienced professional who can develop treatment plans for various conditions, including weight management and chronic disease prevention, while collaborating with a multidisciplinary healthcare team.

Responsibilities

  • Conduct initial nutrition assessments for outpatient clients, reviewing medical history, current lifestyle, dietary habits, and health goals.
  • Create personalized nutrition plans based on assessment findings, addressing specific health conditions, dietary restrictions, and client preferences.
  • Educate patients on proper nutrition, meal planning, and healthy eating habits.
  • Monitor patient progress through regular follow-up sessions, adjusting treatment plans as needed.
  • Offer continued support via one-on-one consultations, phone calls, or email.
  • Work collaboratively with physicians, therapists, and other healthcare professionals to ensure a multidisciplinary approach to patient care.
  • Maintain accurate and up-to-date patient records, including assessments, treatment plans, and progress notes.
  • Conduct group nutrition classes or workshops for patients and the community.

Requirements

  • Bachelor's degree in Nutrition, Dietetics, or a related field (required).
  • Registered Dietitian Nutritionist (RDN) certification (preferred).
  • At least 2 years of experience as a nutritionist or dietitian, preferably in an outpatient setting.
  • Strong knowledge of nutrition science, medical nutrition therapy, and health promotion.
  • Excellent communication skills, with the ability to educate and motivate patients.
  • Ability to create individualized nutrition plans based on client goals and health conditions.
