About the position

The Patient Support Associate role at Syracuse Community Health is designed to assist patients in navigating their healthcare journey. This position focuses on providing education about health services, ensuring patients are connected to necessary resources, and promoting health literacy among diverse populations. The associate will play a crucial role in mitigating barriers to care and enhancing patient access to essential services, thereby contributing to the overall mission of improving community health outcomes.

Responsibilities

  • Assist patients with their healthcare journey by providing education on the importance of keeping scheduled appointments and using My Chart.
  • Liaise with health services to ensure patients receive necessary services including food, housing, and transportation.
  • Administer the SCH Survey tool and educate patients on using kiosks, My Chart, tablets, and telehealth services.
  • Provide excellent customer service and promote health education to patients and staff.
  • Maintain patient charts and ensure timely data entry.
  • Conduct community outreach in health education programs targeting specific populations.
  • Participate in health fairs, community activities, and special events.
  • Assist patients with food insecurity, transportation, and housing needs.
  • Maintain comprehensive documentation for data collection and reporting requirements.
  • Educate and assist patients with setting up and using telehealth services.

Requirements

  • High school diploma or GED is required.
  • Excellent written and verbal communication skills.
  • Ability to teach others and document information accurately.
  • Experience working with diverse populations.
  • Proficiency in Microsoft Office, Outlook, Word, and Excel.
  • Strong multitasking and problem-solving skills.
  • Ability to prioritize tasks effectively.
  • Flexibility and positive interpersonal skills.
  • Knowledge of video conferencing platforms.

Nice-to-haves

  • Experience in healthcare or community service roles.
  • Familiarity with electronic medical records systems.
