About the position

The START Coordinator position at YAI involves supporting individuals diagnosed with intellectual/developmental disabilities (I/DD) and co-occurring behavioral health conditions. Under the supervision of the Clinical Team Leader, the coordinator manages a caseload of 20-25 individuals, conducting evaluations and assessments, providing resources, and coordinating with an interdisciplinary team to support individuals through crises and prepare for discharge. The role requires regular outreach, documentation, and participation in meetings to ensure adherence to the START model.

Responsibilities

  • Maintain an active caseload of up to 20-25 individuals referred for START Services.
  • Conduct regular outreach visits to individuals' homes and vocational settings for evaluations and assessments.
  • Assist with referrals for consultation and treatment, including screening for enrollment in START.
  • Ensure coordination of support meetings and crisis plan development using the START Action Plan.
  • Develop Comprehensive Service Evaluations and Cross-Systems Crisis Prevention and Intervention Plans.
  • Participate in recurring consultation and staff meetings with START leadership and clinical teams.
  • Provide on-call support on a regularly scheduled basis as part of the START team.
  • Complete and maintain all applicable documentation and records in a timely manner.
  • Accurately enter required data into START Information Reporting System and other systems as required.
  • Conduct regular outreach with community partners to maintain relationships.
  • Develop materials and present at Clinical Education Team Meetings as assigned.
  • Participate in the Center for START Services National Network and related events.
  • Complete necessary trainings to ensure compliance with YAI policies and state regulatory agency requirements.

Requirements

  • Master's degree in Mental Health, Psychology, Counseling, Social Work, or other Human Service field.
  • At least two years of experience working with the I/DD population and/or behavioral health needs.
  • Successful completion of START Coordinator Certification training within 12 months of appointment.
  • Ability to provide on-call coverage for crisis services on a recurring basis.
  • Ability to travel regularly across assigned regions for assessments and evaluations.
  • Exceptional interpersonal and problem-solving skills.
  • Strong communication and writing skills.

Nice-to-haves

  • Experience working with individuals with I/DD with co-occurring diagnoses and complex behavioral needs.
  • Experience working with families and social service providers.
  • Verbal and/or written fluency in Spanish; fluency in other languages may be preferred.
