As an Occupational Therapist, you will conduct home visit Occupational Therapy assessments of referred patients, providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation. You will be responsible for creating personalized treatment plans that cater to the unique needs of each patient, ensuring they receive the highest quality of care. Additionally, you will be expected to maintain accurate and timely EMR documentation, utilizing our system, Hello Note, although prior experience with this system is not required. Your role will also involve collaborating with other healthcare professionals to ensure a comprehensive approach to patient care, and you will have the opportunity to work with a diverse population, including both pediatric and geriatric patients.

Responsibilities

  • Conduct home visit Occupational Therapy assessments of referred patients.
  • Provide direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
  • Create personalized treatment plans for patients based on their unique needs.
  • Maintain accurate and timely EMR documentation using Hello Note or similar systems.
  • Collaborate with other healthcare professionals to ensure comprehensive patient care.

Requirements

  • Graduated from an accredited Occupational Therapy (OT) college or university.
  • Unencumbered Occupational Therapy (OTR/L) license for New York.
  • CPR Certification.
  • Proficient in EMR documentation.

Nice-to-haves

  • Experience with Hello Note EMR system.
