As an Occupational Therapist, you will conduct home visit Occupational Therapy assessments of referred patients, providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation. You will be responsible for creating personalized treatment plans that cater to the unique needs of each patient, ensuring that they receive the highest quality of care. Your role will also involve collaborating with other healthcare professionals to ensure a comprehensive approach to patient care. Proficiency in EMR documentation is essential, as you will be required to maintain accurate and up-to-date patient records. We use Hello Note for our documentation, but experience in this system is not required. Additionally, you will need to hold a valid CPR certification to ensure the safety and well-being of your patients.

Responsibilities

  • Conduct home visit Occupational Therapy assessments of referred patients.
  • Provide direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
  • Create personalized treatment plans based on patient assessments.
  • Collaborate with other healthcare professionals to ensure comprehensive patient care.
  • Maintain accurate and up-to-date patient records using EMR documentation.

Requirements

  • Graduated from an accredited Occupational Therapy (OT) college or university.
  • Unencumbered Occupational Therapy (OTR/L) license for New York.
  • Proficient in EMR documentation (experience in Hello Note is not required).
  • CPR Certification.
