As an Occupational Therapist, you will conduct home visit assessments of referred patients, providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation. You will be responsible for ensuring that each patient receives personalized care tailored to their specific needs, which is essential for their recovery and overall well-being. Your role will also involve maintaining accurate and thorough documentation of patient progress and treatment plans, utilizing our EMR system, Hello Note, although prior experience with this system is not mandatory. Additionally, you will be expected to stay current with CPR certification to ensure the safety and well-being of your patients.

Responsibilities

• Conduct home visit Occupational Therapy assessments of referred patients.
• Provide direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
• Maintain accurate and thorough documentation of patient progress and treatment plans using EMR documentation.
• Ensure compliance with all relevant regulations and standards in Occupational Therapy.

Requirements

• Graduated from an accredited Occupational Therapy (OT) college or university.
• Unencumbered Occupational Therapy (OTR/L) license for New York.
• Proficient in EMR documentation (experience in Hello Note is not required).
• CPR Certification.
