About the position

The Care Manager (Generalist) at Volunteers of America Los Angeles plays a crucial role in providing intensive care to program participants who have been involved with the criminal justice system and are suffering from co-occurring disorders. This position requires the Care Manager to meet with participants at least weekly, serving as the primary point of contact to coordinate services and care for assigned clients. The Care Manager is responsible for delivering individualized care to clients with high levels of acuity and co-morbidities, ensuring they have access to necessary service linkages, counseling, treatment planning, and crisis intervention methods. The ultimate goal is to prepare individuals for further success in their lives. In this role, the Care Manager conducts comprehensive screenings and assessments to gather essential information about the clients' functional, environmental, psycho-social, financial, employment, housing, educational, and health needs. This information is used to develop a detailed care plan that addresses the high acuity needs of the clients. The Care Manager sets specific goals for the clients and locates the resources necessary to achieve these goals. Regular follow-ups and progress tracking are essential components of this position, ensuring that both short-term and long-term needs are met effectively. Additionally, the Care Manager facilitates support groups and group education events, providing a platform for clients to share experiences and learn from one another. The role may also require the Care Manager to appear in court or other government agencies as an advocate for clients, particularly in relation to their re-entry into society. This position is vital in helping clients navigate their challenges and achieve a better quality of life.

Responsibilities

  • Conduct comprehensive screenings and assessments with individuals or families to collect relevant information.
  • Develop detailed care plans to meet the high acuity needs of clients.
  • Set goals for clients and locate necessary resources to meet those goals.
  • Provide service linkages and support systems to ensure identified needs are met.
  • Coordinate individualized planning and follow-up with clients to meet their short and long-term needs.
  • Track progress on meeting agreed-upon goals and record all services provided and case notes.
  • Maintain confidentiality of client's data in accordance with HIPAA and other relevant regulations.
  • Facilitate support groups and group education events.
  • Advocate for clients in court or other government agencies regarding re-entry into society.

Requirements

  • Must be able to pass a fingerprint clearance and background check, including criminal history and personal references.
  • Valid California driver's license and reliable transportation are required.
  • Approval from Department of Health Services (DHS) and a TB Test are necessary.
  • Bachelor's Degree in Social Work or a related field is required, or a combination of education and relevant experience totaling 5 years.
  • At least 1 year of case management experience in homelessness, addictions, under-served populations, or prison re-entry program care.

Nice-to-haves

  • 2 years of experience working with the target population is preferred.
  • Familiarity with HMIS and electronic record-keeping is a plus.
