About the position

The Office Base LPN Intake Coordinator at Visiting Redi-Nurse is a full-time role focused on coordinating patient admissions and ensuring quality care delivery in a home health setting. The position requires a self-managed and organized Licensed Practical Nurse (LPN) who is passionate about nursing and capable of multitasking in a busy environment. The coordinator will facilitate communication between various stakeholders, including physicians, insurance companies, and staff, to tailor treatment plans that promote patient independence and recovery.

Responsibilities

• Review appropriateness of services with every referral.
• Utilize knowledge of regulatory (State, Federal and CMS) as it applies to the admission process.
• Maintain knowledge of clinical applications.
• Coordinate admission process by facilitating communication between physician, insurance companies, pharmacies, staff, and other facility services.
• Evaluate staffing in conjunction with nursing coordinators.
• Assign referrals as indicated and assign appropriate staffing level to referral.
• Assure that pending referrals are updated with written documentation.
• Respond to community information calls as needed.
• Coordinate case management with insurance clerk to assure insurance eligibility for services and pre-authorization of care as required by insurance company.

Requirements

• Graduate of an accredited school of Licensed Practical Nursing.
• Current License as a LPN by the state of Florida.
• Some home health experience.
• Good typing/computer skills.
• Excellent communication skills.

Nice-to-haves

• Preferred knowledge of Medicare Regulations.
• 1 year of nursing experience.
