About the position

The On-Site Enrollment Coordinator role at Esrun Health is designed to assist medical practices and patients with in-person enrollments and onboarding to the Chronic Care Management program. This position focuses on proactive patient care for individuals with chronic conditions, ensuring they receive consistent support and follow-up. The Enrollment Coordinator will work closely with Esrun Health supervisors and on-site staff to enhance patient engagement and improve healthcare delivery.

Responsibilities

• Enrolling and onboarding patients onsite to the Esrun Health platform.
• Providing patients with appropriate devices and educating them on their use.
• Ensuring patients can set up their devices correctly at home.
• Meeting with Esrun Health supervisors to optimize day-to-day work.
• Attending regularly scheduled meetings, including bi-monthly staff meetings and monthly one-on-ones.
• Other duties as assigned.

Requirements

• Certified Medical Assistant or experience in a medical office/front desk role.
• Ability to exercise initiative, judgment, organization, time-management, problem-solving, and decision-making skills.
