About the position

The Virtual Registered Nurse (RN) is a key member of the Nursing Care Team, responsible for providing professional nursing services through a computerized and digital platform. This role involves the assessment, planning, implementation, coordination, monitoring, and evaluation of patient care from admission to discharge, ensuring a seamless transition across the continuum of care. The RN utilizes clinical knowledge and critical thinking skills to support the patient's plan of care while monitoring quality metrics and ensuring accurate documentation.

Responsibilities

  • Collaborate and communicate effectively to facilitate optimal patient care.
  • Assess, plan, implement, coordinate, monitor, and evaluate the patient's plan of care.
  • Utilize clinical knowledge and critical thinking skills in case management and adult education.
  • Ensure seamless transitions of care across the continuum.
  • Monitor quality metrics specific to the department and ensure accurate documentation.
  • Perform roles as a mentor, clinician, educator, consultant/collaborator, and navigator.

Requirements

  • Graduation from an accredited professional school of nursing.
  • Valid RN licensure in the applicable state(s) of practice/employment.
  • Minimum of eight (8) years of experience in health condition management, including adult condition management.
  • Experience with Electronic Medical Records (EMR).
  • Valid driver's license where required by assignment.

Nice-to-haves

  • Baccalaureate Degree in the Science of Nursing (BSN) from an accredited school of nursing.
  • Specialty credentialing and educational degree according to clinical nursing practice specialty area.
