About the position

The Virtual Registered Nurse (RN) is an integral member of the Nursing Care Team, tasked with the critical responsibility of ensuring optimal patient care through collaboration, communication, and facilitation via computerized and digital platforms. This role encompasses the comprehensive management of patient care from admission to discharge, requiring the RN to assess, plan, implement, coordinate, monitor, and evaluate the patient's care plan. The RN will utilize their clinical knowledge and critical thinking skills, alongside principles of case management and adult education, to effectively support and coordinate the care plan. A key aspect of this position is to ensure a seamless transition of care across the healthcare continuum, which involves monitoring quality metrics specific to the department and ensuring complete and accurate documentation in the patient record. In addition to clinical responsibilities, the Virtual RN will take on multiple roles including mentor, clinician, educator, consultant/collaborator, and navigator. This multifaceted position requires a strong commitment to patient care and the ability to adapt to the dynamic nature of healthcare delivery. The RN will be expected to work part-time during the day shift, specifically from 7 am to 7:30 pm, and will be part of a team that values effective communication and collaboration to enhance patient outcomes.

Responsibilities

  • Collaborate with the Nursing Care Team to provide optimal patient care through digital platforms.
  • Assess, plan, implement, coordinate, monitor, and evaluate the patient's plan of care from admission to discharge.
  • Utilize clinical knowledge and critical thinking skills to support the care plan.
  • Ensure seamless transitions of care across the healthcare continuum.
  • Monitor quality metrics specific to the department and ensure accurate documentation in patient records.
  • Perform roles as a mentor, clinician, educator, consultant/collaborator, and navigator.

Requirements

  • Graduation from an accredited professional school of nursing.
  • Valid RN licensure authorized in the applicable state(s) of practice/employment.
  • Minimum of eight (8) years of experience in health condition management, including adult condition management.
  • Experience with Electronic Medical Records (EMR).
  • Preferred: Baccalaureate Degree in the Science of Nursing (BSN) from an accredited school of nursing.

Nice-to-haves

  • Specialty credentialing and educational degree according to clinical nursing practice specialty area.
