About the position

The Virtual Registered Nurse (RN) is a key member of the Nursing Care Team, responsible for the optimal provision of care through digital platforms. This role involves assessing, planning, implementing, coordinating, monitoring, and evaluating patient care from admission to discharge, ensuring effective transitions across the care continuum. The RN will utilize clinical knowledge and critical thinking to support patient care and will also monitor quality metrics and maintain accurate documentation.

Responsibilities

• Collaborate and communicate effectively to facilitate optimal patient care.
• Assess, plan, implement, coordinate, monitor, and evaluate the patient's plan of care.
• Utilize clinical knowledge and critical thinking skills in case management and adult education.
• Ensure seamless transitions of care across the continuum.
• Monitor quality metrics specific to the department and ensure accurate documentation in patient records.
• Perform roles as a mentor, clinician, educator, consultant/collaborator, and navigator.

Requirements

• Graduation from an accredited professional school of nursing.
• Valid RN licensure authorized in the applicable state(s) of practice/employment.
• Minimum eight (8) years of experience in health condition management, including adult condition management.
• Experience with Electronic Medical Records (EMR).

Nice-to-haves

• Baccalaureate Degree in the Science of Nursing (BSN) from an accredited school of nursing.
• Specialty credentialing and educational degree according to clinical nursing practice specialty area.
