About the position

The Virtual Registered Nurse (RN) plays a crucial role in the Nursing Care Team, responsible for providing professional nursing services through a digital platform. This position involves collaboration, communication, and facilitation of optimal patient care from admission to discharge, ensuring a seamless transition across the continuum of care. The RN utilizes clinical knowledge and critical thinking to assess, plan, implement, and evaluate patient care, while also mentoring and educating others in the process.

Responsibilities

• Collaborate with the Nursing Care Team to provide optimal patient care through digital platforms.
• Assess, plan, implement, coordinate, monitor, and evaluate the patient's plan of care from admission to discharge.
• Utilize clinical knowledge and critical thinking skills to support the plan of care.
• Monitor quality metrics specific to the department and ensure accurate documentation in the patient record.
• Perform roles as a mentor, clinician, educator, consultant/collaborator, and navigator.

Requirements

• Graduation from an accredited professional school of nursing.
• Valid RN licensure authorized in the applicable state(s) of practice/employment.
• Minimum eight (8) years of experience in health condition management, including adult condition management.
• Experience with Electronic Medical Records (EMR).

Nice-to-haves
