This is an entry level clinical pharmacist position. The clinical pharmacist supports the vision, mission, and goals of YNHHS and the Department of Pharmacy and demonstrates competency in all basic clinical pharmacist skills.

Requirements

  • Graduation from an accredited College of Pharmacy with a B.S. or PharmD degree
  • CT Pharmacist Licensure required, active and without restrictions
  • BLS (Basic Life Support) certified or obtained within 6 months of hire
  • Immunization certified or obtained within 6 months of hire
  • Basic knowledge of pharmacology and therapeutics
  • Excellent verbal and written communication skills
  • Ability to work with others

Nice To Haves

  • Completion of an ASHP-accredited PGY1 pharmacy residency or equivalent experience

Responsibilities

  • Demonstrate and promote CHAMP behaviors
  • Provide excellent customer service to patients and other healthcare team members
  • Provide medication counseling to patients and families
  • Demonstrate effective oral and written communication skills
  • Conduct timely, accurate medication order review considering all patient characteristics
  • Provide medication therapy monitoring for assigned patients
  • Develop an effective interdisciplinary relationship with the health care team
  • Act as a source of medication information for the health care team
  • Ensure clinical continuity where applicable to work area
  • Demonstrate competency in pharmacy automation and technology
  • Oversee sterile and non-sterile compounding to ensure compliance with regulatory statutes
  • Oversee medication distribution/dispensing process in work area
  • Adhere to all departmental policies, procedures, protocols, and guidelines
  • Report medication events and near misses
  • Provide education and training to other pharmacy staff members
  • Participate in professional development activities
  • Contribute to departmental cost-reduction/revenue generation
