About the position

The Outpatient Surgery Coder is responsible for accurately assigning principal and secondary diagnoses and procedures based on thorough reviews of medical documentation. This role ensures compliance with Official Coding Guidelines and federal regulations, while also meeting coding quality and quantity expectations. The coder will work remotely, managing multiple projects and utilizing problem-solving skills to ensure accurate coding prior to billing submission.

Responsibilities

  • Assign principal and secondary diagnoses and procedures by reviewing all documentation in the medical record.
  • Review discharge summaries, history and physicals, physician progress notes, and other medical records to assign appropriate codes.
  • Ensure correct code selection following Official Coding Guidelines and compliance with federal and insurance regulations.
  • Sequence diagnoses and procedures in order of clinical significance for accurate reimbursement under the Prospective Payment system.
  • Review coding for accuracy and completeness before submission to billing.
  • Abstract required medical and demographic information from the medical record and enter it into the system.
  • Ensure all required components of the medical record are present and comply with CMS, JCAHO, and client requirements.
  • Identify incomplete or conflicting documentation and formulate physician queries for clarification.
  • Utilize computer applications and resources essential for efficient coding processes.
  • Meet coding quality and quantity expectations.

Requirements

  • Minimum 2 years of Medical Coding experience required.
  • Experience with Professional Fee Coding.
  • Experience with EHR systems Meditech and EPIC preferred.
  • Accepted certifications from AHIMA or AAPC: RHIT, RHIA, CCA, CCS, CCS-P, CPC.
