The Hepatitis Education Project (HEP) is a nonprofit organization based in Seattle, WA, dedicated to providing support, education, advocacy, and direct services for individuals affected by hepatitis. The Care Coordination Program at HEP is designed to provide direct services to individuals with hepatitis, focusing on client-identified goals and reducing barriers to treatment and care. The Care Coordinator will engage in community outreach to deliver testing, education, referral, and case management services to clients who are disproportionately affected by viral hepatitis, homelessness, and substance use. The role involves working at various community sites, such as low-barrier housing, substance use treatment programs, syringe services programs, and health fairs. HEP's Care Coordination Program is notable for housing one of the first hepatitis C case management programs in the country, which is responsible for identifying individuals living with hepatitis C and linking them to necessary medical care and treatment. In this position, the Care Coordinator will provide viral hepatitis and infectious disease testing, harm reduction services, health education, and comprehensive case management services. The primary focus will be on reducing barriers to hepatitis C treatment and care, with direct service engagement occurring predominantly at designated encampments in South King County and at HEP's fixed site syringe service program located in the International District. The role requires a strong commitment to community outreach and the ability to work effectively with diverse populations in various settings.

Responsibilities

  • Provide viral hepatitis and infectious disease testing to clients.
  • Deliver harm reduction services and health education to individuals at risk.
  • Engage in community outreach to connect with populations in designated encampments.
  • Conduct comprehensive case management services aimed at reducing barriers to hepatitis C treatment.
  • Collaborate with community sites, including low-barrier housing and substance use treatment programs.
  • Link clients to medical care and treatment for hepatitis C.
  • Support clients participating in HEP's syringe service program and opioid use disorder treatment program.

Requirements
