The Outreach Community Health Worker (CHW) will primarily maintain a presence in the community with members of the community to promote, motivate and improve healthy living. The Outreach CHW will assist community members in utilizing resources and accessing services to overcome barriers to meet their health goals. This role is essential in addressing the social determinants of health and ensuring that community members have access to the necessary resources to lead healthier lives. The CHW will engage directly with individuals in various settings, including homes, faith-based organizations, community-based organizations, and homeless shelters, to provide support and guidance in navigating health care services. The Outreach CHW will conduct needs assessments using the Social Determinants of Health Tool to identify barriers that community members face in accessing primary care services. This includes helping individuals with social issues such as homelessness, rental assistance, utility assistance, food security, transportation, childcare services, and substance abuse. The CHW will also play a critical role in scheduling appointments for community members with primary care providers and specialists, as well as assisting them in understanding their insurance eligibility and renewal processes. In addition to these responsibilities, the Outreach CHW will enroll community members in the CHW program for a duration of 30 days, conducting weekly follow-up calls and home visits to ensure that appointments are kept and social needs are addressed. Education for self-management of health conditions will also be provided, helping community members set health goals such as planning healthy meals and finding time to exercise. This position is grant-funded through the end of 2025, with a strong possibility of future extensions, and requires a full-time commitment from 9 AM to 5 PM, Monday through Friday.
