About The Position
At Author Health, we believe in providing compassionate, comprehensive technology-enabled mental health care that meets our patients where they are, particularly for individuals facing serious mental illness (SMI), substance use disorders (SUD), and dementia. Our mission-driven, person-first company is seeking a self-motivated and empathetic Outreach Liaison to join our team. This role is essential to connecting patients with the critical mental health and social services that they need. The Outreach Liaison role requires strong interpersonal skills, the ability to build trust quickly, and a relentless drive to help patients access the care they deserve. As an Outreach Liaison, you will play a pivotal role in Author Health's approach to care by meeting in-person with patients with SMI, SUD and dementia - in the community, at their homes, primary care physician offices, and at local hospitals. Your focus will be on building connections and relationships, and helping these patients and their caregivers schedule and attend Author Health health care appointments.
