About the position

The Outreach Navigator is a full-time position at Liberty Community Services, Inc., focused on assisting homeless individuals and families in New Haven, CT. The role involves building rapport with clients, facilitating access to essential services, and supporting them in transitioning from homelessness to permanent housing. The Navigator will work collaboratively with a team to implement outreach strategies and provide individualized support to clients, helping them overcome barriers and achieve housing stability.

Responsibilities

  • Identify and build rapport with homeless individuals and families living on the street or in vehicles.
  • Facilitate access to basic needs and services at the Resource Center.
  • Assist clients in breaking the cycle of homelessness by moving from the street to housing.
  • Provide individualized client support throughout their journey to permanent housing.
  • Document progress toward desired outcomes, including housing, health, and income stability.
  • Prepare case-related reports including outcomes, successes, and challenges.
  • Provide information, referrals, linkages, and advocacy to assist clients in accessing services and resources.
  • Assist clients with procuring necessary documents and services such as identification cards and social security income.
  • Attend weekly Outreach & Engagement Team meetings and clinical supervision.
  • Network with other agencies and community groups to bring vital services to clients.

Requirements

  • Minimum associate's degree with a concentration in human services.
  • Experience working with persons with mental health and/or substance use disorders.
  • Experience working with the homeless or marginalized population.
  • Proficient with PCs and Microsoft Office Word & Outlook.
  • Understanding of Housing First philosophy.
  • Training in harm reduction, motivational interviewing, and de-escalation techniques.
  • Strong organizational, written, computer, and communication skills.
  • Valid CT driver's license required.

Nice-to-haves

  • Bilingual capabilities are a plus.
  • Experience with employment, housing, and homelessness services strongly preferred.
